As a pedestrian, I always look down the alley before crossing. It's also a good idea to walk on the far side (not right up against the building) of the sidewalk when crossing in front of an alleyway, this gives you a better view down the alley and increases your visibility to a driver. Looking down an alley when I walk is second nature now.

As a driver, I always stop before exiting the alley and crawl very slowly (idle speed) when exiting.

I think its key for everyone to take caution and be aware, drivers and pedestrians.
