Comments:In food establishments, there shall be adequate sewage and waste water disposal facilities that comply with all the requirements of the plumbing section of the Municipal Code of Chicago. WASTE WATER BACKING UP FROM FLOOR DRAIN AND DRAINING ONTO FLOOR WHEN DISH MACHINE AND 3-COMP SINK ARE IN USE. INSTRUCTED MANAGER TO CALL PLUMBER AND REPAIR. PLUMBER ON SITE AT END OF INSPECTION. CRITICAL VIOLATION 7-38-030 CITATION ISSUED.
18. NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS
Comments:All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. FRONT AND REAR DOORS NOT RODENT PROOFED 1/4" GAP BETWEEN FRONT OUTER DOORS AND 1/2" GAP ON LEFT SIDE AND BOTTOM OF REAR DOOR. SEAL ALL OUTER OPENINGS ON DOORS TO PREVENT PEST ENTRY. OBSERVED 10-15 MICE DROPPINGS INSIDE CABINET UNDER BAR, 10-15 MICE DROPPINGS UNDER BAR COMP SINK, 10-15 MICE DROPPINGS ALONG EAST WALL BEHIND CHEST FREEZER AND ALONG SOUTH WALL NEAR BEER TAP, 10-15 MICE DROPPINGS SCATTERED ON TOP OF SEASONINGS, CANNED GOODS AND BULK CONTAINERS IN DRY STORAGE AREA, 10-15 MICE DROPPINGS INSIDE FRYER CABINET, 10-15 MICE DROPPINGS ON FLOOR IN ELECTRICAL CLOSET, 10-15 MICE DROPPINGS ON FLOOR ALONG ICE MACHINE, 10-15 MICE DROPPINGS BEHIND EQUIPMENT IN PAPER GOOD STORAGE AREA. 15-20 MICE DROPPINGS INSIDE GLASS BOWLS AND FOIL PANS, ON PAPER, ETC IN OFFICE. INSTRUCTED MANAGER TO CALL AN EXTERMINATOR FOR SERVICE, CLEAN AND SANITIZE ALL AREAS. SERIOUS VIOLATION 7-38-020 CITATION ISSUED.

Comments:All food not stored in the original container shall be stored in properly labeled containers. COVER AND LABEL ALL BULK CONTAINERS.
31. CLEAN MULTI-USE UTENSILS AND SINGLE SERVICE ARTICLES PROPERLY STORED: NO REUSE OF SINGLE SERVICE ARTICLES
Comments:Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use.INVERT ALL CLEAN MULTI-USE EQUIPMENT AND UTENSILS IN PREP AND DISH WASHING AREAS.
33. FOOD AND NON-FOOD CONTACT EQUIPMENT UTENSILS CLEAN, FREE OF ABRASIVE DETERGENTS
Comments:All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. CLEAN INTERIOR AND EXTERIOR OF ALL FRYERS, FRYER CABINETS, STEAM TABLES, COOKING EQUIPMENT, PREP TABLES, COUNTERS, COOLERS, FREEZERS.
34. FLOORS: CONSTRUCTED PER CODE, CLEANED, GOOD REPAIR, COVERING INSTALLED, DUST-LESS CLEANING METHODS USED
Comments:The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. GROUT LOOSE FLOOR TILES IN BAR, PREP AND DISH WASHING AREAS. FLOORS NEED CLEANING UNDER ALL EQUIPMENT ALONG WALLS AND IN ALL CORNERS IN PREP AND STORAGE AREAS.
35. WALLS, CEILINGS, ATTACHED EQUIPMENT CONSTRUCTED PER CODE: GOOD REPAIR, SURFACES CLEAN AND DUST-LESS CLEANING METHODS
Comments:The walls and ceilings shall be in good repair and easily cleaned. CEILING NEED REPAIR IN DRY STORAGE AREA (PEELING PAINT). SEAL ALL OPENINGS ALONG BASEBOARDS IN DISH WASHING AREA. CLEAN ALL VENTS ON VENTILATION HOOD.
36. LIGHTING: REQUIRED MINIMUM FOOT-CANDLES OF LIGHT PROVIDED, FIXTURES SHIELDED
Comments:Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. REPLACE DAMAGED LIGHT SHIELDS IN PREP AREA.
38. VENTILATION: ROOMS AND EQUIPMENT VENTED AS REQUIRED: PLUMBING: INSTALLED AND MAINTAINED
Comments:Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. REPAIR LEAKING DRAINPIPE AND FAUCET ON 2-COMP SINK. REPAIR LEAK ON SIDE OF EXPOSED SINK. REPAIR LEAKING BACK FLOW DEVICE ON DISH MACHINE.
41. PREMISES MAINTAINED FREE OF LITTER, UNNECESSARY ARTICLES, CLEANING EQUIPMENT PROPERLY STORED
Comments:All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. REMOVE ALL UNUSED EQUIPMENT AND UNNECESSARY ARTICLES IN PREP AND STORAGE AREAS TO PREVENT PEST HARBORAGE.
43. FOOD (ICE) DISPENSING UTENSILS, WASH CLOTHS PROPERLY STORED
Comments:Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. FOOD ITEMS, BEER BOTTLES, ETC MUST BE STORED IN SELF-DRAINING ICE CONTAINERS. STORE WASH CLOTHS IN SANITIZING SOLUTION.
